Transition of healthcare from adolescence to adulthood is defined as the “purposeful, planned movement of adolescents with chronic medical conditions from child-centred to adult-oriented health care”.

It is well recognised that this process requires mutual cooperation between paediatric and adult health care professionals, but also proper structure and training in all phases of the process. Commonly, both paediatric and adult-care professionals make some well-intentioned mistakes that may lead to loss for follow-up, lack of trust, excessive dependence on parents and lack of self-confidence.

One well-recognised problem is the lack of age-appropriate care from both sides, with paediatric healthcare professionals tending to be a bit pampering and adult healthcare provider overestimating self management skills of the 18-year-old patients. Another problem is the forgetting about parents, who need extra care and attention to gradually loosen control of their child who is developing independence skills. Needless to say, good quality young adult care necessitates special commitment from both sides. Many publications have addressed the transition process and published recommendations and guidelines in many chronic diseases that start in paediatric age.

It is globally accepted that a poor transition process may lead to adverse outcomes that may put patients’ life at risk in some conditions that depend completely on the compliance with treatment protocol.

Why apply?

The expected outcome of this school would be the wide implementation and awareness of obtaining the desired skills to hand over and to receive young adults, regardless of whether this process occurs within the same institution or not.

This course is addressed to Paediatric Gastroenterologists and Hepatologists, as well as Adult Gastroenterologists and Hepatologists, AHPs are also welcomed, to provide information and training on the best practices for the handover process, with participation of some successful European programs.

The 2-day course would be held in Porto, Portugal expecting to address 30 delegates from Paediatrics, Gastroenterology, Hepatology  and AHPs to discuss and train in the planning and practice of transition programs. The program will include lectures but also role-model playing and discussion of scenarios within small groups.

Who can apply?

Adult and Paedriatric gastroenterologists, hepatologists. Priority will be given to joint applications. Although separate application is required, please include the name of the other candidate at the bottom of your motivation letter.

AHPs are also welcomed to apply.

We invite you to submit your CV and motivation letter stating your intention to attend the above two key days. We encourage you to submit an abstract of cases, experiences or scenarios that may be of interest for general discussion.

Please do not make travel arrangements before receipt of a formal confirmation of acceptance from the ESPGHAN Office. The number of participants is limited and a selection will be made by ESPGHAN after the deadline. 
ESPGHAN Members may attend two courses per year and non-members may attend one course before becoming members of the society. 

Delegates are expected to attend the full course and arrange their travel according to the course schedule.
